  5. 5For phone/video appointments: provide best contact number/email, be ready 5 minutes before to 30 minutes after booked time, turn off call blocker, sit in a quiet, private space.
  6. 6Bring photo ID to pick up annual medication expense records.
  7. 7If picking up for another person, bring a signed and dated authorization letter and your photo ID.
