This article explains the rights patients have over their health data, and how Cortico helps you apply those rights.
As an initial note, Cortico is currently undergoing significant development to improve these features to provide better control of your heatlh records. Your rights are improtant to us and please let us know at privacy@cortico.health anything we may do to improve.
Accessing Your Records
- Appointments - Sign into Cortico under a specific clinic, and visit Appointments under your “Patient Home” area.
- Files and Documents - (ie, Lab requisitions, lab results, forms). Sign in and go to Files.
- Other History - Sign in. A history of events Cortico is aware of are listed in the HIstory Section.
- Records your can’t see in Cortico. Some records may not have been shared by your healthcare providers. If you need them, message your provider using secure messaging (if enabled) or the clinic’s preferred contact method, requesting the record. Once received, you’ll have it stored in your personal health record area indefinitely.
Sharing Your Records
From your account area, you can currently download files and send them using secure messaging to cortico-enabled clinics. Cortico is currently working on more and better methods to share records.
Correcting Your Records
You can correct demographic information by signing into Cortico, viewing your patient profile and clicking Edit.
Removing/Deleting Your Records
Removing medical records can be complex, because your healthcare providers have generally sworn an oath to prioritize your safety and health over other matters. Removing records risks a patient safety issue since the absence of those records may prevent you receiveing appropriate care sometime in the future. It’s often safest to retain records at any place you received (or may receive) medical care. However, if you do need records removed for some reason, write to both your clinic and privacy@cortico.health requesting the removal of our respective copies to begin the process.
