  1. 1If you have a primary care provider, please contact their office directly.
  2. 2For preventative health screening, call 613-332-1565 ext.321.
  3. 3All appointments for Family Health Team services are booked by the main reception at 613-332-1565 EXT. 220.
  4. 4For Cardiac Rehabilitation, participants require a referral from their primary care provider. Send referrals to the Bancroft Community Family Health Team using fax number 613-332-5541.
  5. 5For Diabetes Education Program, self-referrals and referrals from primary-care providers are accepted for anyone ages 18 and up. To schedule an appointment, contact 613-332-1565 ext. 220.
  6. 6For Pulmonary Rehabilitation, participants require a referral from their primary care provider. Send referrals to the Bancroft Community Family Health Team using fax number 613-332-5541.
  7. 7For Rapid Access Addictions Medicine (RAAM) Clinic, walk-in during open hours (Monday 1:00 PM to 3:00 PM and Wednesday 9:00 AM to 12:00 PM) or call/text 613-403-1733 to make an appointment. You can bring a supportive friend or family member.
  8. 8To become a patient, complete a New Patient Application. If you currently have a family doctor at BCFHT and wish to join the waiting list for a new doctor, you must first DEROSTER from your current family doctor.
  9. 9If you have a family doctor out of town, you are encouraged to keep them while waiting for another doctor.
  10. 10Keep the clinic updated on any changes to your mailing address by calling 613-332-1565 ext. 220 or in person.
  11. 11For medical attention while on the waiting list, consider IDA Bancroft Virtual Walk-in Clinic, Gilmour Medical Clinic, The ER, Ontario Addiction Treatment Centre's (O.A.T.C), Midwifery Services of Haliburton-Bancroft, Diabetes Education Support (for Type I/II diabetes), 211 Ontario, or Health Connect Ontario (811).
