Patient Instructions
  1. 1Fill in the online secure questionnaire and submit it to our office (1 form for each traveller).
  2. 2Bring any immunization records that you have.
  3. 3Bring a list of any medications you are taking or have taken in the past three months.
  4. 4For TB Test: Payment is required for each TB Skin Test Step.
  5. 5For TB Test: Return 48 hours after your TB Skin Test to have your TB test read (Thursdays).
  6. 6For TB Test: Complete your TB Skin Test PRIOR to receiving a live immunization such as MMR, Varicella, Oral Typhoid, Yellow Fever, or wait 4 weeks before starting TB Skin Testing.
  7. 7For TB Test: Wear a clean facemask and notify our office if you have/had cold symptoms in the last 5 days prior to attending our clinic.
  8. 8For TB Test: Bring any paperwork with you at the time of your appointment.
