Patient Instructions
  1. 1If you have Washington Apple Health Insurance and want to enroll in our services, you can walk into our Bremerton office at 5455 Almira Drive NE from 8 a.m. to 4 p.m. Mondays through Fridays.
  2. 2First visits may take up to two hours.
  3. 3If you are a parent hoping to enroll your child in services, your child must be present for the visit.
  4. 4If you wish to receive services at our South Kitsap office, please talk to the Access Department about your options.
  5. 5If you need to be seen outside of regular office hours, please inquire at intake or ask our Call Center, 360-373-5031, what other options are available.
  6. 6For children with an open dependency case with the Department of Children, Youth, and Families, their social worker must either be present during the first appointment, or must contact our Access Center at 360-373-5031 so arrangements to sign a consent to treat form, releases of information, and provision of the Court report regarding the dependency can be made prior to the First Appointment.
