Instructions

  1. To be eligible for OHIP Funded Physiotherapy, patients must be referred by a physician or nurse practitioner for an acute injury or worsening chronic condition and have a valid Ontario Health (OHIP) Card.
  2. Patients eligible for physiotherapy funding via a work injury or Motor Vehicle Accident insurance are not eligible for OHIP covered therapy.
  3. Clients having hip, knee, or shoulder replacement surgery are to attend post-operative therapy at the hospital where the surgery was done; if they wish to attend physiotherapy locally, the client is responsible to cover the costs.
