  2. If the questionnaire is not complete, you will be asked to remain outside until it is completed.
  3. If you answer “yes” to any of the questions, please stay home and self isolate until you can prove a negative test or asymptomatic for min 14 days. Your appointment can be rescheduled and no fees will be charged.
