  1. 1When calling for your first appointment, please tell us the reason for your visit and have your Alberta Health Care number ready.
  2. 2Family members may come with you, but if they need to be seen by the doctor, we ask that they make a separate appointment.
  3. 3Before your appointment, please check in with reception so we can update your information and inform the module staff that you have arrived.
  4. 4We will need: Your current address, Your phone number, Emergency contact number, Two pieces of ID, including one photo ID.
  5. 5Call at least 24 hours in advance if you cannot make your appointment.
  6. 6If you’re going to be late for your appointment please let us know.
  7. 7If multiple appointments are missed without cancellation, you may be asked to seek medical attention at another facility.
  8. 8If you are a new patient and you miss your first appointment, another appointment will not be made with any physician in the clinic in the future.
  9. 9Bring all your medications in the original containers to your visits at the clinic.
  10. 10Book the next visit before leaving the clinic, to ensure that the timing of your next visit will prevent running out of medication.
  11. 11Please do not assume “no news is good news” regarding test results. Partner with your physician in planning what follow-up is needed.
  12. 12If you have financial concerns, please discuss these charges with your doctor.
