Patient Instructions
  1. 1Report work accidents and injuries to employer as soon as possible.
  2. 2Tell doctor about work injury for WCB reports.
  3. 3Complete the Worker’s Report of Injury form and submit to WCB.
  4. 4Attend regular appointments as recommended by Physical Therapist to avoid discontinuation of WCB claim.
  5. 5Report car accidents and any injuries to insurance company within 10 days or as soon as possible.
  6. 6See a primary health practitioner (Physiotherapist or Family Physician) for initial assessment and treatment after a car accident.
  7. 7Complete a Notice of Loss & Proof of Claim form (AB-1) and return to insurance company within 10 days (for Diagnostic & Treatment Protocol) or 30 days (for Section B Protocols) from the date of accident.
  8. 8Exhaust extended health benefits coverage prior to accessing coverage under Section B for car accident injuries.
