Patient Instructions
  1. 1If your child has not been seen for an appointment within the last 12 months, a new referral will be required to reactivate their medical chart.
  2. 2Schedule your next appointment before leaving the office.
  3. 3Outdoor shoe removal is required; consider bringing clean indoor footwear.
  4. 4If you fail to show, are more than 10 minutes late, or fail to cancel a minimum of 24 hours prior, a fee will be required before rescheduling.
  5. 5Print, complete, and bring an intake form to your visit.
  6. 6Do not offer primary care.
  7. 7Not a walk-in clinic.
  8. 8Require a referral from your doctor.
  9. 9Please try to always make an appointment with the same Pediatrician.
  10. 10Food/Drinks are not permitted in the clinic.
  11. 11Strollers cannot be accommodated inside the office; leave them in your vehicle or outside.
  12. 12Medication is prescribed until the next follow-up; prescriptions are not refilled by telephone or fax.
  13. 13Appointments are required for completion of forms, letters, and doctor’s notes; a fee will be determined at the time of the visit.
  14. 14Not all injections and immunizations are covered by Alberta Health; you will be responsible for payment.
  15. 15The transfer of a Medical chart usually involves a fee.
  16. 16Only cash, cheque, or e-transfer are accepted for services not covered by Alberta Health.
  17. 17Office will contact you if abnormal lab results are received; reception staff cannot give or interpret lab results unless directed by a Pediatrician.
  18. 18Do not communicate via e-mail or text regarding health issues.
  19. 19No photography or filming on the premises.
  20. 20Arrive 15 minutes early to complete paperwork.
  21. 21Present your child’s valid Alberta Health Care card upon arrival.
  22. 22Bring a list of current medications and any related information.
  23. 23Write a clear list of questions and refills required.
  24. 24New patient appointments need to be confirmed a minimum of 24 hours prior or they will be cancelled.
  25. 25If the appointment is for school or attention issues, complete the assessment at www.ADHDratingscales.com before your appointment.
  26. 26Bring report cards or reports/letters from the school or previous assessments for school/attention issues.
