  6. 6If requesting a complete physical or pap smear, inform the receptionist as these require a longer booking period
  7. 7If you cannot make your appointment, you must change or cancel it 24 hours in advance, otherwise a no show fee of $50-$80 will be automatically invoiced
  8. 8Test results are only communicated if abnormal; staff cannot provide test results over the phone
  9. 9If you would like a copy of your result, there will be a charge for photocopying and Admin time
  10. 10For referrals, either the office or the specialist office will contact you with an appointment time
  11. 11Patients are responsible for paying in advance for any forms needing to be filled out by the doctor
  12. 12Inform the receptionist of any forms needing to be filled out prior to seeing the doctor
  13. 13Ensure your file is kept up to date with current phone number, cell phone number, address, and emergency contact information
  14. 14Inform the receptionist of any changes to your personal information upon arrival at the clinic
