Patient Instructions:
  • Prescription refills may take up to 2 weeks to be completed. Call your pharmacist for refill requests.
  • Consult Reports/Imaging Results may take up to 2 weeks or longer. Do not call the office asking for results prior to this.
  • Please be patient when calling into the office due to higher than normal call volume.
  • For forms to be completed, call the office and book an appointment to discuss the form with your family doctor. Complete the 'to be filled out by applicant' section. Bring 2 copies of the form, one blank and one completed by you.
  • If you have had recent blood work, imaging or a biopsy done, your doctor will contact you with your results once they are available. Ensure your email is up to date and check it regularly. The office does not contact patients if results are normal.
  • Contact your pharmacy for all prescription renewals. Allow 5 days for completion of renewals.

Parking is located at the front of the Trailside Medical Building.

How to Book:

Call on the morning you would like a same-day appointment. Phones are answered starting at 9:00am on weekdays. For pre-booked appointments, call the office. For after-hours clinic, call (519) 787-2940 up to 30 minutes in advance of the scheduled opening time.
