  9. 9Do not consult for chronic problems (non-recent onset), medication prescription renewal (including narcotics), blood test prescriptions, result retrieval, sick leave/work stoppage follow-up, ADHD/behavioral disorder/school difficulty evaluation, chronic pain (without significant recent increase), non-urgent post-operative follow-up, or routine Pap tests in minor emergency clinic.
  10. 10If no appointments are available online for an emergency, present to the hospital emergency department or call 9-1-1.
  11. 11Present proof of identity when retrieving documents.
  12. 12Provide written and signed consent for document retrieval for a third person aged 14 and over.
