For Geriatric Inpatient Consultation Service, acute care Physicians and Nurse Practitioners can refer. For Geriatric Outpatient Rehabilitation Service, Physicians and Nurse Practitioners from hospital or community can refer using the NESGC Coordinated Access Referral form. For Geriatric Mental Health Outreach Service and Behavioural Support Outreach Service, community and hospital-based care providers, self-referrals, physicians, and nurse practitioners can refer.
