  1. 1Complete the "Your Symptoms Matter" symptom screening tool at home within 24 hours of your appointment.
  2. 2Bring your drugs/medications in their original containers, inhalers and injectables, including any natural, herbal or traditional medicines.
  3. 3Bring your Ontario health insurance card.
  4. 4Bring your status card number (if applicable).
  5. 5Bring a note pad to write down information.
  6. 6Bring payment for parking (cash, debit, credit).
  7. 7Bring a snack and drink (especially if you are diabetic).
  8. 8For computer/or telephone appointments in your home, set up in a quiet space with a good Wi-Fi connection.
  9. 9Do a test or a trial run of the computer system before your visit.
  10. 10It is important to make sure you have a family member present with you during your first visit with the oncologist.
