  3. 3Bring your OHIP Health Card and all other insurance information.
  4. 4If you have a pacemaker, bring your pacemaker card.
  5. 5Do not bring any valuables with you as the hospital cannot be responsible for lost or stolen personal items.

  2. 2It is recommended to park in the designated Patient Parking Lots 1 and 2 and proceed to the Medical Outpatient Entrance - level 0.
  3. 3If Parking Lots 1 and 2 are full, park in Lots 3 or 4 and proceed to the West 5th Street entrance and take the stairs or elevator to the Medical Outpatient Department - level 0.

Referral Process: Most elective surgical patients will be required to attend the Pre-Op Clinic. Referrals for Robotic Thoracic Surgery should be faxed to the Thoracic Surgery Common Referral Office. Bariatric surgery referrals are accepted by a referring physician through the Bariatric Registry.
How to Refer: Fax referral form for Robotic Thoracic Surgery. For Bariatric Surgery, physicians refer patients to the centralized referral portal.
