Madigan Army Hospital

• Active duty service members should report to the Cardinal/Swan Team during sick call hours (7:30-8 a.m.) to receive a same day appointment with a Department of Family Medicine provider. Appointments are on a first come, first served basis and cannot be made over the phone.
• All service members will sign their name in the SICK CALL Record book to verify date, time, and presence.
• Use the MHS GENESIS Patient Portal to request medication renewals. Include the medication name(s) in your message.
• Alternatively, call the Clinic Nurse Line at 253-968-0770 and leave a detailed message, including your name, date of birth and DOD ID number, and the medication you wish to refill.
• Initiate medication refill requests a week prior to your medication running out. Requests require three business days for processing.
• If you need an urgent refill, contact Pharmacy directly at 253-968-2510.
• Call the Madigan refill line at 253-968-2999 to ensure all available refills have been used before proceeding with other instructions.
• For well child immunizations for children under 2 years of age, a well-child examination appointment is required before immunization.
• If a home pregnancy test is positive, call the OB Care Coordinator at 253-968-0676 to schedule an initial appointment.
• Keep your family's information up to date in DEERS.

Madigan Family Medicine is located on the ground floor of the Medical Mall. From the Madigan Army Medical Center Gate (Interstate 5 Exit 122) continue on Jackson Ave. to the second traffic light (on base) and turn right at Beaumont Ave.

Patients can schedule appointments by calling the PIRMAC at 800-404-4506. Active duty service members can also report for sick call appointments on a first-come, first-served basis.
