  2. 2For Substance Use Disorder Assessments, walk in Monday through Friday between 8:00 am and 2:00 pm.
  3. 3Bring a photo ID for assessment appointments.
  4. 4A urine sample will be collected for a drug screen as part of the assessment process and may be requested intermittently throughout care.
