  2. 2Notify reception staff upon arrival for nurse injections (flu shot, allergy shot, TB test, etc.) and plan to wait 20 minutes after for observation.
  3. 3Bring a valid health card (OHIP card) for each visit; otherwise, services will be charged.
  4. 4Do not use email or text for sensitive medical information regarding sexually transmitted diseases, AIDS/HIV, mental health, developmental disability, or substance abuse.
  5. 5Inform the provider of any types of information you do not want sent via electronic services.
  6. 6Follow up if an electronic communication requires a response and you haven't received one within a reasonable time.
  7. 7Do not use electronic communication for medical emergencies or time-sensitive matters.
  8. 8Reasonably limit or avoid using an employer’s or other third party’s computer for electronic communication.
  9. 9Inform NHC of any changes in your email address, mobile phone number, or other account information.

  11. 11Review all electronic communications to ensure they are clear and all relevant information is provided.
