Patient Instructions
  1. 11 issue per visit only; if you have more, you will need another appointment on a different day.
  2. 2Login at 9AM every Monday-Friday to book SAME DAY afternoon appointments (Noon-4PM).
  3. 3Book 24 hours in advance for NEXT DAY morning appointments.
  4. 4Check Google for updated clinic hours.
  5. 5In-person visits are required to discuss results.
  6. 6Notify of cancellation at least 1 hour BEFORE your appointment to avoid a $50 no-show fee.
  7. 7If late without calling/emailing within 10 minutes of appointment, a $20 late fee applies.
  8. 8Fees must be paid before being seen.
  9. 9It is the patient's responsibility to check health card validity and coverage.
  10. 10Expired/invalid/no coverage health card incurs a $60 fee (refundable if valid HC# provided within 3 months).
  11. 11Visitors to Canada or those without coverage pay $125.
  12. 12If you cannot communicate in English, you must be accompanied by an interpreter/translator or prepare your problem/symptoms in English.
