Instructions

  1. Patients with out of province health cards will be required to make a payment to Middlefield Medical Centre and then will be provided with a reimbursement form and a receipt of payment so that they can be reimbursed from their province of health coverage.
  2. Travelers with private insurance or uninsured patients will be charged a flat fee of $75 at registration along with any additional fees as required for procedures. A receipt of payment will be provided.
  3. Completion of a travelers insurance form is a separate charge above and beyond the above visit fee.
