Patient Instructions
  1. 1Call or visit the health centre office to book an appointment.
  2. 2Appointments are typically booked one to two weeks out.
  3. 3Patients transferring their care to the medical centre can request the medical centre staff to request their medical records from previous family doctors or specialists.
  4. 4For dental appointments, call or visit the health centre. Appointments with the dentist are available every second Tuesday or Friday of the month (rotating). Dental hygiene appointments are available every Thursday.
  5. 5For COHI team appointments, call or visit the health centre and complete the consent form provided.
  6. 6For blood collection, appointments are required and available on Tuesdays from 8 am - 11:30 am. Appointments are booked on a first-come-first-served basis and are scheduled ten minutes apart. If fasting is required, appointments will be booked as early as possible. If bloodwork is urgent, the centre will try to accommodate or refer to the Elmsdate blood clinic or the Colchester East Hants Health Centre (Truro Hospital).
  7. 7To participate in the Elders group, contact the Elders Program coordinators.
  8. 8To access home care services, call or visit the health centre and ask to speak with the home care team.
  9. 9For child and adult immunizations, contact the community health nurse to book appointments.
