  2. 2For access to records of a deceased patient, fill out the form and return it to the facility that holds the record.
  3. 3Requests for records must be sent to the facility where care was received.
  4. 4For care received at Hôpital Notre-Dame before November 27, 2017, requests must be made to Centre hospitalier de l’Université de Montréal (CHUM).
  5. 5For birth records from Hôpital de la Miséricorde, send a written request to CHSLD du Manoir-de-l'Âge-d'Or, including biological parents' names, date of birth, and a copy of valid ID.
  6. 6For access to records from Centre de protection de l'enfance et de la jeunesse, include copies of valid ID.
  7. 7For users 14 years and older, include a copy of valid ID. If a parent/guardian makes the request, include their valid ID, the user's valid ID, and a signed consent from the user.
  8. 8For users under 14 years, the request must be made by the mother, father, or legal guardian, including a copy of their valid ID.
  9. 9Payments for services must be made before receiving care (some exceptions apply).
