1. 1To take an appointment for blood tests, you must come in person to the Clinic front desk with your request, at Ash or Centre Street.
2. 2Before coming to the Clinic for your blood test, you must make an appointment.
3. 3For doctor appointments, call the Clinic at 514-937-9251 and dial 3 at the exact time (Mondays at 11 am for Monday afternoon, Mondays at 6:30 pm for Tuesday morning, Wednesdays at 6:30 pm for Thursday morning).
4. 4If the line disconnects for doctor appointments, call the Primary Care Access Point (GAP) at 811, option 3.
5. 5To cancel an appointment, call 514 937-9251 and dial 2 at least 24 hours before your appointment.
6. 6Bring your medicare card and medication list to your appointment.
7. 7Leave forms that need filling out with the secretaries at the reception desk after signing them.
8. 8If your medicare card has expired more than 6 months ago, been lost, stolen or damaged, a $25 fee applies for a new card. Payment by check or money order only; no cash accepted.
9. 9Cover your mouth and nose with a scarf, cloth, handkerchief or mask when visiting.
10. 10Come alone OR accompanied by one person.
11. 11Wash your hands at the entrance and respect the 2 meters (6 feet) distance.
12. 12Do not enter the Clinic more than 5 minutes before your appointment.
