Referral Process: Access to the PCU is often facilitated through the Emergency Room, dependent on bed availability. Transfers can be arranged with the unit's Patient Care Coordinator once a bed is available. Palliative physicians may meet patients in their current unit for assessment.
How to Refer: Let the care team know if a palliative care consult is desired. For transfers from the community, speak with the health-care team or call the home and community care access line in the area. Health-care professionals can also fax a completed home and community care referral form to the local community office.
