Patient Instructions
  1. 1Bring another form of payment as some insurance companies or policies may not have direct billing available.
  2. 2Arrive 10-15 minutes before your appointment to fill out appropriate intake forms.
  3. 3Patients are required to wear a face mask (medical or non-medical) to enter the clinic.
  4. 4Patients will be required to wash hands or hand sanitize immediately upon entry.
  5. 5Patients will not be permitted to attend if they have presented with any COVID-19 symptoms in the last 14 days or have been in contact with anyone exhibiting symptoms in the last 14 days.
  6. 6For Motor Vehicle Accident patients, bring a valid claim number, insurance adjuster's name and telephone number.
  7. 7For WSIB patients, notify employer, initiate a claim with WSIB, and obtain a claim number.
