  1. Book a 15-minute appointment, in-clinic for new concerns or follow-up visits. Online booking is not available for physicals, well-baby visits, or procedures.
  2. For urgent issues in the next 2-3 days, appointments longer than 15 minutes, if you do not have an OHIP number, or need an appointment with an allied health provider, call the clinic.
  3. For prescription renewals, ask your pharmacy to fax a medication renewal request. Do not call the clinic for updates on prescription renewals.

  1. Bathurst site is located at the southwest corner of Bathurst Street and College Street.
  2. Garrison Creek is located on the north side of St. Clair Avenue West, east of Oakwood Avenue at Robina Avenue and St. Clair Avenue WeSt.

Bathurst site serves communities from Bloor Street to Lake Ontario, Dufferin Street to Yonge Street. Garrison Creek site serves communities from Eglinton Avenue West to Bloor Street, Keele Street to Spadina Avenue.
