Patient Instructions
  1. 1Bring valid health card.
  2. 2Wear appropriate clothing (shorts for lower extremity injury, sleeveless top for upper extremity injury).
  3. 3Bring any reports of investigations already done.
  4. 4Bring a copy of the referral from family physician (if applicable).
  5. 5Call the clinic to book with a doctor.
  6. 6A referral is required to be seen by a Sport Medicine Physician.
  7. 724 hours notice is required to cancel all appointments, otherwise a $50.00 no show fee will be charged.
  8. 8Missed appointments for Concussions or Orthopedics will be charged $100.00.
  9. 9Physiotherapy assessment is required on first visit, for new injury, or to re-assess existing injury post 60 days.

  1. 1Located in the Ice House arena building on the campus of Carleton University.
  2. 2If entering off Bronson Ave, stay left on University Drive.
  3. 3Turn at the first left into parking lot number 5, or park at meters on University Dr. Go left after the main entrance at the Sport Medicine Clinic sign and follow the ramp to the clinic.
