  1. 1A referral is required before patients are admitted to TOHRC’s treatment programs.
  2. 2For patients currently admitted to a hospital, referrals should be sent through Ocean eReferral using the listing “The Ottawa Hospital – Inpatient Sub Acute Care TOH Rehabilitation Centre” for Acquired Brain Injury (ABI) Service, Locomotor Care /Complex Orthopedics, Neuromuscular Care.
  3. 3For patients currently admitted to a hospital, referrals should be sent via fax for Amputee Rehabilitation (Dr. Nancy Dudek) and Respiratory Rehabilitation (Dr. Jackie Sandoz).
  4. 4Patients being referred directly from the community must first be assessed by a The Ottawa Hospital Rehabilitation Centre Physiatrist in clinic. Please fax a referral to 613-733-8336.
  5. 5For patients not currently admitted to a hospital, a referral for outpatient Physiatry consultation should be sent through Ocean using the listing “The Ottawa Hospital Physiatry Consult – TOH Rehabilitation Centre” for Acquired Brain Injury, Amputee Rehabilitation, Musculoskeletal Rehabilitation, Neuromuscular Rehabilitation, Spasticity Management, Electromyography.
  6. 6Referrals for Respiratory Rehabilitation (Dr. Jackie Sandoz) should be faxed to CANVent: 613-736-9054.
  7. 7If you are on the waiting list you will be contacted as soon as a bed is available and an admission date will be set. Specific instructions for preparation for your inpatient stay will be given at this time.
  8. 8If you have any questions regarding the admission process, please contact the Admitting Department at 613-737-7350 ext. 75508.
  9. 9If an issue related to your TOHRC treatment arises and your family physician cannot manage it, you can call and speak with the nurse, as indicated on your discharge plan.
  10. 10Family members and other caregivers will be taught the skills necessary to properly and safely assist patients and will also be given the opportunity to discuss the disability’s impact on them and how they are coping.
  11. 11Weekend visits at home are encouraged as trial visits to identify issues that still need to be addressed before final discharge.
