Patient Instructions
- 1Always have your health card (or UHIP number if you’re an international student) and student card handy when booking and attending your appointment.
- 2If you can’t submit an assignment or complete an evaluation due to an absence from at least one of your courses, you must complete the declaration of absence from an evaluation form.
- 3If you are unable to attend your scheduled appointment, contact the Centre right away at 613-562-5498 or send an email to health@uottawa.ca.
- 4Any person experiencing flu-like symptoms must wear a mask in the Student Health and Wellness Centre.
