  1. 1If a patient is more than 10 minutes late for their appointment, then their appointment will be cancelled and rescheduled according to availability.
  2. 2Provide at least 24 hours cancellation notice if unable to attend an appointment.
  3. 3Test results will ONLY be provided to the patient and no one else (unless prior written consent is provided by the patient).
  4. 4For urgent and semi-urgent matters, contact the office directly at 905-885-6391.
  5. 5To secure a same-day appointment for urgent care, contact the office promptly at 9:00 am.
  6. 6For after-hours clinic, you MUST call the relevant office beforehand.
  7. 7Prescriptions for narcotics or other controlled substances will NOT be issued by the after hours clinic physician.
  8. 8Uninsured services are NOT covered under OHIP and it is the patient’s responsibility to pay for these services.
  9. 9All forms will be completed through an in-person or virtual appointment with the Physician. Forms dropped off at the office will NOT be completed without an appointment.
  10. 10Require at least 2 weeks notice to complete forms.
