  1. 1New students are encouraged to submit immunization documentation prior to New Coug Orientation or the beginning of their first semester.
  2. 2Graduate students are encouraged to submit immunization documentation prior to arriving on campus.
  3. 3Ensure the file size of uploaded immunization records is not too large.
  4. 4Ensure no special characters are in the file name of uploaded immunization records.
  5. 5Add each dose of vaccinations, not just the most recent dose.
  6. 6Ensure name and birthday are on every page of records.
  7. 7Do NOT consolidate records; upload all records even if from different locations.
  8. 8Handwritten records may be denied without a clinic stamp or medical signature.
  9. 9When uploading proof of T-spot and QuantiFERON Gold tuberculosis testing, ensure date, location, and result are on records.
  10. 10When uploading proof of skin test, include date and time the test was placed and read, clinic's location, and test result.
  11. 11Keep an eye on the patient portal for communication regarding additional information needed.
  12. 12Complete tuberculosis screening questions on the patient portal.
  13. 13TB testing must be completed in the last 6 months and be a US-based test.
  14. 14If receiving a vaccination at CHS, inform healthcare provider if you prefer information not to be uploaded to WAIIS.
