  2. 2For new patients without insurance, a prepayment of $100.00 is required prior to each visit.
  3. 3Please let us know at least 24 hours ahead of time when you cannot keep your appointment to avoid a minimum $90.00 charge ($155.00 for complete physicals) for “no-shows” or late cancellations.
