Patient Instructions
  1. 1Arrive 5-10 minutes early for your appointment.
  2. 2If you arrive late, you may not be seen.
  3. 3To cancel appointments within 24 hours, send an email to info@ccfp.ca with your name, physician, and date/time of appointment.
  4. 4If you miss your appointment or cancel within one hour, you will be charged a fee of $50.
  5. 5Missed circumcision or vasectomy appointments will be charged $150.
  6. 6If you have symptoms of respiratory illness (cough, fever, sore throat, congestion), you will be asked to wear a mask in the clinic.
  7. 7If you do not have a valid provincial Health Service Card, you will be expected to pay for any services received at the time of service.
