Instructions

  1. Schedule follow-up visits at the same time as evaluation as provider schedules fill quickly.
  2. Suggests scheduling 1 visit per week for 4 weeks, or as far out as preferred.
  3. This can be adjusted to your Plan Of Care after your Evaluation.

Getting Here

  1. The entrance to the parking garage under the South Lake Union clinic is off of John Street.
  2. There is an elevator in the garage that will take you to the lobby of the building.
  3. Turn left when you exit the lobby, and then turn right when you get to Yale Ave. The entrance to the clinic will be at the corner of Yale Avenue and John Street.
