  2. 2Previous medical records must be sent to the office prior to registration.
  3. 3Ensure child has their first dose of MMR vaccine if eligible.
  4. 4Download and complete the New Patient Registration Packet forms prior to the first visit.
  5. 5Forms can be filled out electronically using Acrobat Reader or a web browser with a PDF plug-in, then sent via the NSP Patient Portal (My Kids Chart).
  6. 6Do NOT send completed forms via email.
