Instructions

  1. Complete new client paperwork prior to your first appointment. If via telehealth, mail or fax forms. If in-person, arrive 15-20 minutes early.
  2. Bring a complete list of all prescription medications you're currently taking.
  3. Bring your ID and insurance card.

Getting Here

  1. When you arrive, please park in the client parking lot on the west side of the building, right next to the clinic.
  2. ACP's Minneapolis location is now home to a Genoa Pharmacy!
