Patient Instructions
  1. 1Don’t forget to keep your family’s information up to date in DEERS!
  2. 2If you or your child are immunized off post, please provide a copy of your vaccine record to your provider and care team so we can update your immunization record.
  3. 3Everyone, including children, must have their DOD ID number when presenting for a test.
  4. 4Ensure that you notify your chain of command and self-isolate while awaiting your test result.
  5. 5If your test is positive, you are to isolate for a five-day period in accordance with Force Health Protection Guidance Supplement 15 (revision 3).
  6. 6Continue to monitor your symptoms and be sure to seek medical attention for any trouble breathing, chest pain, or confusion.
  7. 7Stay in a separate room from other household members, if possible.
  8. 8Don’t share personal household items and wear a well-fitting mask when around others.
  9. 9Ventilate the space as much as possible.
  10. 10It is imperative that personnel utilize a well-fitting mask for an additional 5 days after leaving isolation when around others.
  11. 11You must stay in isolation beyond five days if fever, shortness of breath, or severe fatigue persist. Please remain in isolation until you no longer have fever for more than 24 hours and any remaining symptoms are resolving.
