- 7Arrive 15-30 minutes before appointment time to fill out registration and enrollment forms.
- 8For group workshops or individual services by the interdisciplinary team, registration with a physician is not required.
- 9Answer COVID-19 screening questions upon arrival for in-person appointments.
- 10Temperature will be taken and recorded prior to appointment.
