Patient Instructions

  2. 2Check your email from us to see which Nurse Practitioner's online schedule to book your appts under.
  3. 3You must arrive 15 minutes before your scheduled appointment time to register and test your vitals.
  4. 4If you arrive more than 10 minutes after your scheduled appointment time, or if you cancel your appt with less than 24hrs' notice, you will be charged a $60 penalty fee and refused to be seen, nor rebook with the clinic, til the penalty fee is paid.
  5. 5Book an in-person appointment to discuss only ONE issue.
  6. 6Wear your own mask if you have a cough/cold/fever or signs of infection.
  7. 7If this is your 1st appt it will only be a "meet and greet" and no specific medical concerns will be discussed.
  8. 8You must book subsequent appts to discuss any specific medical concerns and ONLY ONE ISSUE PER APPT.
  9. 9To cancel a booked appt: Email or call the clinic, with more than 24hrs' notice.
  10. 10For 19 specific conditions, walk into the clinic at 400 Adelaide Street East and go directly to the Pharmacy counter with your OHIP card, or call them on 647-557-6957. They may even be able to prescribe for you over the phone.
