  3. 3Patients will only be contacted if test results require a change in management; otherwise, book an appointment to discuss results.
  4. 4Sick notes are an uninsured service.
  5. 5Driver's License Medicals cost $200; an Urgent Driver's License Medical is $300. Fees must be paid before being seen.
  6. 6If you do not have a provincial medical card, you will need to pay for your visit.
  7. 7Travel insurance is not accepted; pay for the visit and present the receipt to your insurer.
  8. 8If you just moved to Alberta, apply for an Alberta Health Care Card within 3 months.
