  1. 1Fill out the Patient Intake Form online before the appointment, or arrive 30 minutes early to complete it in person.
  2. 2The appointment can take up to 1.5 hours.
  3. 3For upper limb testing, wear a T-shirt/tank top.
  4. 4For lower limb testing, wear/bring shorts.
  5. 5Avoid putting lotion or cream on the body 24 hours prior to the appointment.
  6. 6Do not bring children to the clinic.
  7. 7Confirm attendance to the appointment 1 week in advance.
  8. 8Provide a minimum of 3 business days notice to reschedule, otherwise a $250 fee may apply.
  9. 9Email or fax any imaging and tests (MRI, CT Scans, X-Ray, EMG/NCS) completed prior to the appointment.
  10. 10Arrive 30 minutes early to look for street parking.
  11. 11If more than 15 minutes late, the appointment will be rescheduled with a $250 fee.
  12. 12If you do not show up, your appointment will be rescheduled with a $250 fee.
  13. 13English Language Learners must be accompanied by a translator; otherwise, the appointment may be rescheduled with a $250 fee.
  14. 14You may be asked to remove clothing, jewelry, hairpins, eyeglasses, hearing aids, or other metal objects.
  15. 15You will be asked to lie down for the test.
  16. 16If arms or legs are too cold, they may be warmed up prior to testing.
  17. 17You can request to stop the testing at any time if you feel uncomfortable.

Enable Medical Referral Information

Referral Process: All types of adult referrals for nerve conduction studies & electromyography are welcome, except for patients who require facial testing. Consultations include a full medical history, neurological exam, and focused musculoskeletal exam when a musculoskeletal mimic is suspected. The diagnosis and treatment plan will be discussed with the patient at the end of their appointment. The consultation note will be forwarded to the referring physician within approximately 1 week. The clinic diagnoses neuromuscular abnormalities, aids with prognostication, and suggests next steps for investigations and treatment.
How to Refer: Referrals are accepted via fax.
