Referral Process: Clients referred must be currently receiving community mental health services within VCH. Referrals are managed by Vancouver Coastal Health's Central Access and Discharge (CAD) Team.
How to Refer: VCH providers can download the TMHSU Assertive Community Treatment (ACT) Team Intake Process document. Non-VCH providers should connect with the client's Community Mental Health Team providers to discuss ACT referrals.
