Patient Instructions
  1. 1For medical emergencies, please call 911 or proceed to the nearest Hospital.
  2. 2Completion of the patient enrollment form does not confirm a doctor-patient relationship.
  3. 3A valid OHIP card must be presented at each visit to receive medical care.
  4. 4Patients will be billed directly for appointments if an expired or invalid OHIP card is presented.
  5. 5Payment is required at the time of service for medical services not covered by OHIP.
  6. 6Each individual patient is required to complete and submit a separate Patient Enrollment Request Form.
  7. 7Certain healthcare providers at our facility exclusively see adult patients (18 years of age and older).
  8. 8For travel medicine, book a minimum of 6 weeks in advance of departure.
  9. 9Travel advice is NOT covered by OHIP; consultation fee reimburses the healthcare provider and nurse.
  10. 10Costs for vaccines will be billed separately by your pharmacy.
  11. 11Medical information and test results will not be provided via email or telephone.
