Patient Instructions
  1. 1Masks are required in the office if you have respiratory symptoms or fever.
  2. 2Allow 7 days for prescription renewals during summer and mid-winter holiday season, otherwise 4 working days.
  3. 3Inform the clinic if you are seen in an emergency department.
  4. 4Book an appointment to review test results ordered by other doctors outside of the clinic, specifying the test.
  5. 5Arrive 5-10 minutes early for scheduled appointments.
  6. 6Call to cancel appointments as soon as possible; less than 24 hours notice or more than 10 minutes late may result in a missed appointment fee.
  7. 7Update your Ontario Health Card version code and expiry date with the office.
  8. 8Bring your valid Ontario Health Card to each visit.
  9. 9Update contact information (name, address, phone number, email) with the office.
  10. 10For urgent medical concerns during evening/Sunday urgent care hours, call the same day to be seen.
  11. 11For urgent concerns outside of office and on-call hours, call 811 to speak with a registered nurse.
  12. 12Call 911 or go to the nearest Emergency Department for medical emergencies.
  13. 13For flu vaccine, advise the clinic if received through a pharmacy.
  14. 14Stay home and keep children home when sick.
  15. 15For children with respiratory symptoms, call the family doctor if fever lasts 72 hours or longer, fever returns after going away, child is unusually irritable, earache lasts more than 48 hours, or not eating/drinking.
  16. 16Call 911 or go to the Emergency Department if worried child is seriously ill, infant younger than 3 months has a fever, child is struggling to breathe, or concerned about dehydration.
