Centre for Family Medicine Referral Information

Referral Process: Referrals to the KW4 Regional MINT Memory Clinic can only be made by physicians practicing in Kitchener, Waterloo, or the townships of Wilmont, Woolwich, and Wellesley. All Regional Memory Clinic referrals must be sent to the Clinical Intake Team - Specialized Geriatric Services (SGS) for centralized and proper processing.
How to Refer: Fax referral form to SGS. The Executive Director can be contacted via telephone or letter for patient complaints.
