Instructions

  1. Check with your specific provider to determine the amount of coverage you have for each session and the total amount of coverage you have for each benefit or calendar year.
  2. At the end of each session, you will receive a receipt that includes the psychologist’s certificate number, the service provided, and the amount paid and this receipt can be submitted to your insurance company for re-imbursement.
  3. Any fees that are not covered by insurance can be claimed as a medical expense on your annual income tax return.

  1. Located on the fourth floor (Suite 402) of 22 King Street South in Uptown Waterloo.
  2. Free 2-hour parking is available very close to the front entrance in city lots like Station Lot (20 Regina Street South) and Waterloo Town Square North and South Lots.
  3. Paid full-day parking is available in city lots behind the building or beside Waterloo Town Square.
