Eden Health Care Services Referral Information
Referral Process: Hospital-based services include both inpatient and outpatient services. Inpatient services are for individuals admitted for at least an overnight stay. Outpatient services are for individuals living in the community not admitted to the hospital. Referrals to Outpatient Services are made by treating psychiatrists or primary care providers by faxing a referral letter.
How to Refer: For outpatient services, fax a referral letter to 204-325-8429. For Community Mental Health services, call 1-888-310-4593. For General Practitioners to connect with the psychiatrist on call, contact Eden Mental Health Centre at 1-866-559-2555 or 204-325-4325.
