  1. 1Ask your family doctor for a referral for specialist appointments.
  2. 2Call the office to schedule weekly UVB light therapy treatment appointments.
  3. 3Call the clinic for more information on aesthetic services.
  4. 4Schedule follow-up appointments by calling the clinic.
  5. 5If you haven’t seen Dr. Silver for sometime, a new referral from your family physician may be required.
  6. 6If you haven’t seen Dr. Van De Velde in over 2 years, a new referral from your family physician will be required.
  7. 7For wart treatments ONLY, patients can call directly without a referral.
  8. 8Patients interested in cosmetic services can call directly for an appointment with the cosmetic director.
  9. 9Request a referral from your family physician, nurse practitioner, walk-in clinic physician, or another specialist.
  10. 10Provide advance notice for cancellations to avoid a $50-150 fee for missed appointments.
  11. 11You may ask your pharmacy to fax a refill request for current prescriptions.
  12. 12Do not have cosmetic procedures or use certain products while pregnant or nursing.
  13. 13Avoid significant sun exposure 3 weeks prior or 3 weeks after laser treatment.
  14. 14Do not receive laser treatment if currently receiving UVB light therapy or on photosensitive medication.
  15. 15Before hair removal treatments, do not wax, sugar, pluck, or have electrolysis in the treated area for 4-6 weeks.
  16. 16Medication may be prescribed prior to laser treatment if you've had cold sores in the past.
