Yes. Build custom diagnostic and administrative forms with single-choice, multiple-choice and free-text questions, and show patients messages based on their answers. Yes. Through Cortico’s task menu, clinics can request that patients upload documents or photos — such as insurance or identification — back to the clinic securely as part of the intake process.

No. Cortico matches a returning patient to their existing chart before the form opens, so what you already hold on them — contact numbers, email, health card and insurance details — comes back pre-filled for them to confirm or correct rather than enter again. A new patient fills those in once, during registration, and that is what every later visit starts from. It is easiest to see on a demo with one of your own forms.

On every EMR, what arrives is a note or a document — Cortico does not write form answers into discrete chart fields. Beyond that it differs. On Oscar-family systems (OpenOSP, WELL OscarPro) patient uploads go straight to the e-chart. On Practice Fusion, completed forms, uploads and signed consent file as PDF documents attributed to the provider on the appointment — readable in the chart, but not individually reportable inside Practice Fusion. On Profile (Intrahealth) file attachments are manual. On MPeer, file upload to the EMR is not supported. On Accuro, attachments land in the Documents, Generated Letters, Clinical Notes, Forms and Labs folders. Filing the form document into the chart is a per-clinic setting on Premium; with it off, the completed form is emailed to the clinic instead.

US practices ask about eligibility first: Cortico collects an insurance or ID card as an upload, but does not run US insurance eligibility or benefits verification. Raise it on the demo call if that is a hard requirement.
